Setting Up Your Profile

When you log in for the first time, you’ll be asked to choose the service types you offer. Mark the checkboxes next to the services you provide, then click "Save" to continue.

1. Editing Account Details

  • Navigate to the "Account" tab located at the bottom-right.

  • Tap Contact Information.

  • This is split up into 3 sections and allows you to add more about your business.

  • Tap each category to view the current information and to update your profile with new or additional information.

2. Service Types

To update your service types offered, follow these steps:

  • Navigate to the "Account" tab then tap "Service Types."

  • Select/Deselect service types as needed to reflect what you offer.

  • Tap "Save."

3. Contracted Pricing

  • Review the previously agreed upon pricing we have on file for you. If you find a mistake, email support@checksammy.com for assistance in changing your contracted pricing.

4. Vehicles

Adding a New Vehicle

  • Tap "Account" and then tap "Vehicles."

  • Tap the "+" button at the top right of the screen.

  • Select the type of equipment you are adding.

  • Select either Vehicle or Trailer.

  • Select the type of vehicle it is. (i.e., Pickup Truck, Cargo Van, etc.)

  • Enter the required information: Equipment Name, Make, Model, Year, Color, License Plate Number, and VIN Number. (All other fields are optional.

  • Tap Next.

  • Enter the dimensions of your vehicle and indicate what unit of measurement you're using. (i.e., feet, meters)

  • Tap Next.

  • Select each capability that applies to this vehicle.

  • Tap Next.

  • Review the entered data to check for any mistakes made, then tap Save.

Managing Existing Vehicles

  • To "Edit" or "Delete" an existing vehicle tap the vehicle card and select the pencil icon to edit the vehicle or the trash icon to delete it. If you select edit, make any changes needed and tap "Save." If you select delete, you will be asked to confirm the deletion.

5. Uploading Provider Documentation

Driver's License Verification (Veriff©)

  • Navigate to the "Account" tab located at the bottom-right.

  • Tap "Driver License," then tap "Let’s Go."

  • Follow the prompts to upload the front of your driver’s license, scan the barcode on the back, and capture a selfie for verification.

Proof of Insurance

  • Navigate to the "Account" tab located at the bottom-right.

  • Tap "Compliance," and three more options will appear:

    1. Proof of Insurance

      • There are 3 main types of insurance documents that you can submit -

        • General Liability Insurance

        • Auto Insurance

        • Workers Compensation Insurance (If available)

      • Tap the "+" button in the respective section to upload each document and additional fields will be displayed.

        • Insurance Company - Enter the name of the insurance company you hold the policy with.

        • Policy Holder - Enter the name of the person who holds the policy.

        • Issue At - A calendar will be displayed. Select the effective date of your current policy.

        • Expiration At - A calendar will be displayed. Select the expiration date of your current policy.

        • Coverage Amount - Tap the drop-down and select the currency type that is used on your current policy and then enter the amount covered.

        • Upload File - Tap the document icon to open your file explorer on your mobile device and select the file containing your corresponding insurance policy.

        • Tap "Save" to save the document to your profile.

      • You may add as many additional insurance documents as needed by tapping the "+" icon at the top-right of the screen and follow the steps above for each.

    2. Tax Information

      • Enter your tax ID number.

      • Upload a PDF of supporting documents that include your tax identification number.

      • Tap “Save.”

    3. State Permits

      • Tap “+” at the top-right.

      • Enter the State Permit Name.

      • Upload a PDF copy of the permit.

      • Tap “Save.”

You must have CheckSammy listed as additionally insured for work performed on their behalf. All COI's must be uploaded as a PDF. No other file type is accepted.
